Job Description
We are looking for a special kind of person - a St Vincent's kind of person. Someone who puts compassion, connection and caring for other people at the heart of everything they do. As a Care Partner, you will play a pivotal role in ensuring a high standard of care is maintained at all times within the Aged Care/Home Care space.
Duties
- Coordinating all aspects of Support at Home Packages and private clients within the Melbourne location
- Optimising care within clients identified care needs (liaising with Clinical and Allied Health and key stakeholders)
- Responsible for ongoing care planning, assessment and reviews
- Involvement in recruitment process for staffing requirements
- Manage a team of Support Care Workers
- Assist scheduling based on service needs
- Answer new potential client enquiries regarding available service provision and pricing
Things that work for us
- Desirable Cert III in Individual Support/Aged care (minimum) or Cert IV in Aged Care, RN, higher or similar
- Minimum of 2 years' experience in Home Care case management (essential)
- Work collaboratively with the Operations Manager for the effective management of the programs
- Have shared responsibility to achieve and maintain quality improvement activities, including the community service quality reporting
- Provide advanced level of case management and coordination of service provision to clients
- Prepare and Monitor client budget utilisation identifying underspends/overspends
- Escalate and work with the Operations Manager to manage Serious Incident Response Scheme Reporting (SIRS) reportable to the Commission.
- An ability to support the well‑being of the client through the skilled, caring and efficient execution of required duties
- An ability to observe and report changes in the client's status
- An understanding of Aged Care Standards and consumer rights and responsibilities
- An understanding of the new Aged Care Reforms
- Great time management, bedside and communication skills
- Strong English written and verbal communication skills
- Successful completion and maintenance of Key Personnel Checks.
