Their NDIS Plans Are you an experienced Support Coordinator, Specialist Support Coordinator or Social Worker who is passionate about helping people with disability navigate complex situations, access the right supports and achieve meaningful outcomes? Caring Arms Australia is looking for confident and experienced professionals who can manage their own caseload, undertake complex assessment and coordination, navigate health and social service systems, and build trusted relationships with participants, families, community organisations and professional stakeholders.

For our Specialist Support Coordination, we are particularly interested in qualified Social Workers with experience supporting people with complex disability, psychosocial, health, housing or social support needs. About the Role You will work with NDIS participants, including people with complex disability and psychosocial needs, to understand their circumstances, implement their NDI plans, resolve barriers and connect with appropriate funded, mainstream and community supports. This is a full-time position.

Depending on your qualifications and experience, responsibilities may include: Managing a caseload of NDIS participants, including participants with complex and changing support needs Conducting individual and family interviews and assessments to identify the nature and extent of a participant's disability-related, social, health, housing, welfare and community support needs

Helping participants and families understand their NDIS plans, budgets, available supports and service options Acting as a facilitator between participants and relevant community, disability, health, and social services Assessing and identifying appropriate resources across health, welfare, recreation, housing,



employment and other community services

Connecting participants with suitable NDIS-funded, mainstream, community, and self-help services Assisting participants to understand and work through complex problems by providing information, exploring options, facilitating communication and referring them to appropriate services Acting as a mediator where required between participants, families, providers, community organisations and other stakeholders to assist in resolving service barriers or concerns

Supporting participants to work towards their goals and build greater independence, choice and control Working closely with families, guardians, allied health professionals, hospitals, behaviour support practitioners, accommodation providers and community organisations Cooperating with community organizations, social agencies and voluntary groups to improve participant access to existing services and identify opportunities for new or improved supports

Supporting participants through complex circumstances including hospital discharge, accommodation transitions, SIL/SDA pathways, service breakdowns and significant changes in support needs. Monitoring plan utilisation, service delivery, participant outcomes and emerging risks. Identifying funding gaps, service gaps and barriers affecting participant outcomes.

Preparing reports, assessments, recommendations and supporting evidence for NDIS plan reassessments and other relevant processes. Contributing to the development and implementation of service improvement initiatives, pilot projects and community-based approaches that respond to participant and organisational needs. Assisting with research, evaluation and service-development activities aimed at improving participant outcomes and addressing identified community needs

Contributing skilled knowledge and frontline experience to the analysis, development, promotion and implementation of organisational practices and social policies relevant to disability and community services Maintaining accurate case notes, assessments, reports and compliance documentation Specialist Support Coordination:

For participants experiencing significant complexity or barriers within their support environment, the role may also involve Specialist Support Coordination. This may include: Completing more detailed assessments of a participant's support environment and identifying barriers to successful plan implementation

Coordinating multiple health, disability,



housing, behavioural and community service systems Supporting participants where there is a risk of service breakdown, hospitalisation, homelessness or unsuccessful accommodation placement Facilitating multidisciplinary stakeholder meetings and complex case conferences

Working with participants, families and professionals to develop practical strategies to address complex social and service-related issues Assisting to establish sustainable support arrangements and build the capacity of the participant and their support network Providing professional recommendations and evidence to support future planning and service decisions

About You: You are an experienced, organised and dependable professional who understands how to translate an NDIS plan and broader social support needs into practical, coordinated support.

You communicate clearly, exercise sound professional judgement, follow through on agreed actions and remain focused on achieving meaningful outcomes for the participant. You will ideally have: Demonstrated experience in NDIS Support Coordination, Specialist Support Coordination, Social

Work or complex case management Preferably two or more years of relevant experience Strong knowledge of NDIS plans, funding, plan implementation and mainstream/community service systems

Experience managing your own participant or client caseload Confidence supporting people with complex disability, psychosocial and changing support needs Ability to assess client needs and identify appropriate health, housing, welfare, employment and community resources

Strong skills in case assessment, problem solving, mediation and stakeholder engagement Strong written communication, case documentation and report-writing skills Ability to work collaboratively with families, guardians, health professionals, community organisations and government or social service agencies

Current Driver Licence and reliable vehicle Current NDIS Worker Screening Check Current

Working with Children Check Completion of the NDIS Worker Orientation Module Experience supporting participants with complex physical disability, psychosocial disability, SIL, SDA, hospital discharge, behaviour support, guardianship matters or Specialist Support Coordination will be highly regarded.

Qualifications Relevant tertiary qualifications in Social Work, Psychology, Allied Health, Community Services or a related discipline are highly regarded for Support Coordination positions.

Candidates with a Social Work qualification should clearly identify their qualification, professional experience and complex case-management experience in their application. Location-Wyong,NSW

Salary - $81,000 – $95,000 per year + Superannuation, Job Type- Permanent,
