Job Description

About the Role

As the successful applicant, you will become part of a growing multidisciplinary team at a critical time in the history of St Vincent’s Hospital Melbourne, as we continue to transform traditional inpatient care towards a ‘home first’ approach, including hospital-level care to people living in residential aged care facilities.

Duties

- Provide optimal physiotherapy management to patients through effective assessment, treatment, evaluation, liaison, and discharge planning.
- Provide high quality clinically appropriate, comprehensive allied health therapy to patients and carers in their own homes, aligning with the patient’s expressed goals of care and healthcare needs, and considering the effects of the patient’s health issues on their life and wellbeing.
- Demonstrate sound clinical knowledge of evidence based practice, research and new developments.

Classifications

Classifications for this position will range from VB1 – VB4 ($86,730 - $99,699 per annum, pro rata for part time).

Your Contribution

- You will have broad and relevant clinical experience ideally including community based programs.
- You have demonstrated ability to work effectively within a multidisciplinary team.
- You have effective interpersonal skills including problem solving, negotiation and teaching.
- You are experienced in working with patients with multiple comorbidities that impact safety, function and independence.
- You are passionate about continually improving and have solutions focused approach to improve service delivery.
- Current National criminal history check, or willing to obtain.

Acute @Home is a forward-thinking and dynamic directorate, comprised of inter-linked services - Residential In-Reach (RIR), Hospital in the Home (HITH), District Nursing (DN) and Palliative Care @Home (PC@H) - with all programs aimed at supporting patients to return home successfully and earlier from campus-based care. The Acute @Home physiotherapist works as an integral member of medically-led multidisciplinary teams, to plan and provide care to HITH and RIR patients in their residential aged care facility, home, HITH @The Cottage, place of work, place of study, and HITH clinic. The patient population supported by RIR and HITH is across the adult lifespan, from diverse cultural backgrounds and with a broad spectrum of clinical conditions.
