Job Description

Position Description Job Title Community Support Worker (Disability): Full-time Position Summary Our organization is seeking an experienced, motivated Community Support Worker to join our growing team full-time. This is an excellent opportunity to work with a dedicated organization delivering person-centered support to individuals with disabilities, promoting independence, community participation, and well-being.

The role primarily delivers direct client care while also supporting service coordination and mentoring of support staff to maintain service quality and compliance. About the Employer We are a Disability and Aged Care service provider based in Sydney.

We started providing care services in 2018 as an NDIS-registered service provider. We value and practice genuine person-centered care for clients with disabilities and older Australians. Our goal is to nurture genuine connections with people receiving and providing care, enhancing the lives of individuals with disabilities and older Australians through high-quality support.

Adhering to the NDIS and Aged Care Practice Standards, we prioritize compliance and quality while delivering safe and culturally competent care to our clients.

Key Responsibilities The successful candidate will be responsible for delivering the following duties - 1.

Core Duties

- Provide direct personal care, including assistance with hygiene, dressing, grooming, and mobility
- Support clients with daily living activities and independent living skills
- Assist clients in accessing community services, healthcare, housing, and social support




- Facilitate community participation and inclusion in social and recreational activities
- Provide emotional support and guidance to improve well-being and resilience
- Monitor and document client progress, maintaining accurate and compliant records
- Liaise with families, carers, and allied health professionals to ensure coordinated care
- Advocate for clients’ rights, dignity, and independence

2. Disability Support Functions

- Deliver NDIS-compliant disability support services
- Support clients with behavioral, physical, and communication needs
- Implement positive behavior support strategies where required
- Assist with:
- Medication (where authorized)
- Manual handling
- Risk management and safety
- Facilitate capacity-building programs, including life skills and community access
- Encourage client independence and goal achievement as per care plans

3. Team Leadership & Coordination

- Provide on-the-job guidance and mentoring to support workers
- Assist with training and induction of new staff
- Support team coordination, and help recruit and manage daily care rosters ensuring continuity of care
- Act as a first point of escalation for routine staff or client issues
- Participate in case reviews and team meetings
- Support compliance with NDIS Practice Standards and organizational policies.

Reporting Structure

- Reports to:



Operations and Compliance Manager
- Supervisory Scope:
- Provides informal or limited supervision only
- No full managerial control over staff

Qualifications & Experience Essential

- Certificate III or IV, Diploma or higher in:
- Individual Support (Disability)
- Community Services
- Disability Support
- Minimum 2–3 years relevant work experience in disability services working at a senior level capacity managing roster and team leadership.
- Experience in community or disability support work
- Knowledge of community support services and government-funded programs.

Licenses & Checks

- NDIS Worker Screening Check
- Police check
- First Aid & CPR Certificate
- Valid driver’s license (if role requires)

Skills & Competencies

- Strong understanding of person-centered care
- Knowledge of the NDIS framework and compliance requirements
- Excellent communication and interpersonal skills
- Ability to maintain documentation and reporting standards
- Robust organizational skills and
- Problem-solving and conflict resolution ability
- Capacity to mentor others without acting as a formal manager
- Ability to work independently and as part of a multidisciplinary team

Employment Conditions

- Employment Type: Full-time (minimum 38 hours/week)
- Remuneration: $80,000 – $85,000 per annum plus superannuation
- Location: In Community / Client residence within Australia
- Work Schedule: Rotating roster, including weekdays and weekends as required.
