The Role
We are seeking an experienced Team Leader to provide leadership, guidance and operational support across our Home Care and NDIS services.
This is an exciting opportunity for a people-focused leader who is passionate about delivering high-quality, person-centred care while supporting and developing a team. You will act as a key link between management, employees, consumers and their families, ensuring services are delivered efficiently, compliantly and in line with individual care needs.
Reporting to the Support at Home Manager, you will oversee day-to-day team performance, client care planning, compliance, service delivery and continuous improvement.
Key Responsibilities
- Provide day-to-day leadership, guidance and support to team members, fostering a positive, collaborative and high-performing culture.
- Recruit, onboard, train and supervise team members, ensuring they have the skills and resources required to deliver excellent care.
- Monitor team and individual performance, provide coaching and feedback, and support ongoing professional development.
- Set and monitor KPIs and support employee engagement and accountability.
- Conduct client assessments and develop, implement and review personalised care plans and NDIS plans.
- Work closely with consumers, families, care providers and healthcare professionals to establish and achieve individual care goals.
- Build strong relationships with consumers and families and act as a key point of contact for enquiries, concerns and service-related matters.
- Ensure client records, care plans, progress notes and other documentation are accurate, current and completed within required timeframes.
- Ensure services are delivered in line with relevant legislation, policies, accreditation standards and funding requirements.
- Support monthly compliance audits and ensure delegated actions are completed.
- Identify and manage potential risks, safety concerns and service issues.
- Drive continuous improvement initiatives to enhance care quality, processes and service delivery.
- Support the day-to-day operations of the business unit and report key findings and issues to the Support at Home Manager.
- Prepare and contribute to weekly operational reports.
- Foster effective communication and collaboration across care providers, clients, families, Care Managers and other stakeholders.
- Identify opportunities to strengthen existing client relationships and support business growth where appropriate.
About You
- Demonstrated experience in home care, aged care, care management, NDIS or a related healthcare setting.
- Previous experience in a leadership, supervisory or management role with Strong organisational, problem-solving and time-management skills.
- Excellent communication and relationship-building skills, with the ability to engage effectively with consumers, families and stakeholders.
