JOB DESCRIPTION
Job Description
You’ll support adults with chronic and complex health needs to build independence and improve their quality of life through practical, person-centred care.
You’ll partner with clients, carers and other health professionals to deliver evidence-based services in home and community settings. This includes:
- Completing comprehensive assessments, care planning and home visits
- Recommending assistive technology, equipment and home modifications
- Supporting clients to navigate aged care and community funding pathways
- Delivering education, coaching and self-management strategies
- Collaborating with a multidisciplinary allied health team
- Participating in supervision, case reviews and continuous improvement initiatives
- Maintaining high-quality documentation and contribute to service outcomes
