Job Description

Monash Health seeks a Social Worker (Grade 3) to join the Intensive Community Treatment team for Older Adults across seven community sites in two regions, with hybrid working where appropriate.

You will perform comprehensive mental health assessments, deliver psychosocial interventions, and coordinate care with families, carers and agencies, supporting complex presentations and discharge planning.
