Bolton Clarke is seeking a Care Partner to serve as the designated care manager under the Support at Home Program. You will oversee care management activities to help older people live safely and independently at home, coordinating with clinical teams when complex care planning is required.
You will collaborate with participants to develop individualised care plans, liaise with GPs and allied health providers, and ensure audits and quality standards are met while maintaining accurate care notes
