This role involves providing practical, person-centred support to individuals with disability and psychosocial needs within their homes and local community. Based in Brisbane and Surrounding Areas - Northside, Wynnum, Ipswich & Logan. You will assist participants to strengthen life skills, maintain safe living environments and access meaningful activities that promote wellbeing and inclusion.
Key responsibilities
- Providing structured in-home support with daily living tasks and routines
- Supporting individuals with psychosocial disabilities, mental health, substance use or transitioning from incarceration
- Supporting skill development and independence within the home environment
- Assisting with community access, appointments and social participation
- Implementing individual support plans and behaviour support strategies
- Monitoring wellbeing and reporting changes in needs
- Liaising respectfully with families and informal supports
- Maintaining accurate documentation and progress notes
About you
- Qualification in Community Services, Social Work, Disability, or related field (or working toward) - desired
- Experience supporting people with disability or psychosocial needs
- Solid communication skills and professional boundaries
- Ability to work independently and follow structured support plans
- Current driver's licence and reliable vehicle
- NDIS Worker Screening Check (or willingness to obtain)
- Full working rights in Australia
