Registered Nurse – Community / Home‑Based Care

We are seeking experienced and patient‑focused Registered Nurses to deliver in‑home education and sub‑cutaneous therapy support for patients prescribed specialist medications.

This is a flexible, community‑based opportunity suited to nurses who enjoy autonomy, patient education, and working one‑on‑one in the home setting.

About the Role

In this role, you will support patients and/or their carers through:
Education and training on self‑administered sub‑cutaneous injections
In‑home sub‑cutaneous infusions, including pre‑ and post‑observations
Documentation, reporting, and adverse event monitoring in line with program requirements

Visit Expectations
Patient contact to be made within 48 hours of referral to arrange a mutually convenient visit
Visit duration:
Injection training: approx. 1.5 hours
Infusion visits: approx. 2.5 hours
Completion of visit documentation and time recording within 24 hours via a secure online portal
Prompt reporting of any adverse events, incidents, or spills in accordance with protocols

Training Provided
Comprehensive online training program covering:
Program overview




Disease and medication education
Medication administration
Pharmacovigilance and adverse event reporting
Standard operating procedures

Essential Requirements
Current Registered Nurse registration (AHPRA)
Current CPR certification
Demonstrated experience with Sub‑cutaneous injections AND IV cannulation
High attention to detail and strong clinical documentation skills
Excellent communication and interpersonal skills
High level of computer literacy and confidence using online portals
Flexible availability (days and times)
Access to a protected, roadworthy vehicle
Current unrestricted Australian driver’s licence

Desirable Experience
Community or home‑based nursing experience
