About the Role

Enhance Hope Services Pty Ltd is seeking compassionate, reliable and person-centred Support Workers to provide high-quality support to NDIS participants and help them live safely, independently and with dignity.

Key Responsibilities
Provide personal care and daily living assistance.
Support participants with household tasks, meal preparation and community access.
Assist with mobility and transfers as required.
Provide social and emotional support.
Follow individual support plans, care plans and risk management procedures.
Assist with medication where appropriately trained and authorised.
Maintain accurate progress notes and required documentation.
Report incidents, changes in participant needs and concerns to the appropriate supervisor.
Maintain participant privacy, dignity, choice and independence.
Follow NDIS Practice Standards, EHS policies and workplace health and safety requirements.

Pay: $28.00 – $36.00 per hour

Expected hours: No less than 8.0 per week
