Job Description

ForHealth is seeking a Registered or Enrolled Nurse to join the Deception Bay Medical Centre on a part time basis, specialising in Chronic Disease Management and preventative healthcare.

You will work with GPs, nurses and allied health specialists to coordinate personalised care for patients with chronic conditions, empowering them through education, SMART goals and ongoing monitoring. Join a supportive team dedicated to improving long-term patient outcomes.
