Open Minds in Penrith seeks a Change Facilitator to help clients map out what matters and develop shared change plans. You will build relationships, connect people to community resources, and ensure high-quality, compliant service delivery through reporting and progress reviews.

You will be trained in Motivational Interviewing and work with clients, families and services to support independence and recovery outcomes.
