Job Description

The Smith Family is seeking an On PAR Case Worker to provide holistic case management for students and families within the On PAR program in Mt Druitt, NSW. This is a temporary full time position through December 2027 with hybrid working options.

You will maintain a caseload, collaborate with schools and local service providers, manage data for evaluation, and support the design and delivery of client-centred services. A Social Work degree and NSW Driver's Licence are required.
