Job Description

VMCH invites applications for a Case Manager – Carer Services to support carers across Victoria in a community setting. This fixed-term role focuses on assessing needs, developing goal-directed support plans and connecting carers with the right services at the right time.

Using a person-centred, strengths-based approach, you will provide tailored advice, advocacy and referrals to help carers build resilience and sustain their wellbeing.
