Job Description
Intereach is seeking a Care Partner – Support at Home to join our team in Bendigo or Echuca on a 12-month contract. You will conduct assessments, develop person-centered care plans, coordinate services and ensure safe, high-quality care for participants.

Role requires at least two years in home-based aged care or a related qualification, with robust knowledge of aged care systems and excellent communication and decision-making skills.
