As the Care Manager Team Leader, you will support and supervise Care Managers to deliver high-quality, relationship-centred community-based services. Reporting to the Business Operations Manager, you will help ensure care management practice is consistent, compliant and aligned with legislative, regulatory and organisational requirements.

Key Responsibilities
n
n
Support consistent delivery of high-quality, relationship-centred services across the client journey.
n
Provide day-to-day supervision and guidance to Care Managers across care planning, documentation, risk monitoring and service coordination.
n
Act as the first point of escalation for complex client matters, safeguarding concerns and service risks.
n
Conduct regular audits of care plans, client reviews, budgets, risk assessments and documentation to maintain compliance and audit readiness.
n
Lead capability development through onboarding, mentoring, performance support and targeted education for Care Managers.
n
n
About you
n
n
Experience leading or supporting teams in aged care, community services or a similar environment.
n
Strong people leadership, with the ability to coach, mentor and support teams through change.
n
A practical understanding of care management, quality, compliance, risk and safeguarding. SIRS knowledge would be highly regarded.
n
Robust organisation and problem-solving skills, with confidence managing competing priorities and complex client matters.
n
A collaborative, resilient and values-led approach, with a genuine commitment to clients, families and your team.
n
n
A tertiary qualification in Nursing, Allied Health, Social Work or a related discipline will be highly regarded.
n
