The Residential Care Line Service based in Sir Charles Gairdner Hospital (SCGH), is Western Australia's only State-wide, Nurse Practitioner led outreach service providing support to residential aged care staff to provide care in place.
Our passionate team of nurses work collaboratively with residential aged care home (RACH) staff and practitioners, providing clinical excellence, expert knowledge and education to empower RACH staff to improve the quality-of-life of older people living in residential care, supporting their choice to receive care in their 'own home'.
