Together. is looking for a Health Linker to help people access the health, community and social supports they need to improve their wellbeing and work towards their recovery goals.This is a people-focused role for someone who understands that positive health is about more than appointments and treatment. Sometimes, the right connection to a community group, creative activity, social support or practical service can make all the difference.About Better. Together.Better. Together. is a free mental health service for people aged 18 and over across the Murrumbidgee region.Our multidisciplinary teams work alongside general practices to provide person-centred, recovery-oriented and holistic support. We look beyond someone's immediate health needs to understand the wider factors that can affect wellbeing including loneliness,
social isolation and access to community resources.The Health Linker plays a key role in turning those connections into meaningful opportunities for people to participate, belong and thrive.Where you come inThe role is also about building connections between services by working closely with GPs, other health professionals, community organisations, families and carers to create coordinated pathways and reduce the barriers people can face when trying to access support.You will have...Experience working with people experiencing mental health or psychosocial challengesA genuine commitment to person-centred, recovery-oriented practiceThe ability to listen, understand and work alongside people as the experts in their own livesConfidence navigating health, community and social service systemsA collaborative approach to working with multidisciplinary teams and external stakeholdersStrong organisation and documentation skillsThe ability to build community networks and identify opportunities that reduce isolation and increase participationYou will need:Tertiary qualification in Social Work, Mental Health, Nursing, Allied Health, Aboriginal Health, Community Services or a related field (completed or in progress), or equivalent relevant experience in the sectorStrong understanding of recovery-oriented,
