Our Hobart office is a multidisciplinary team of OTs, Speechies, Physios, EPs and BSPs, supporting clients across the NDIS and Support at Home programs.
n
WhatYou'llDo
n
Based at our Hobart office,you willprovide OT services to Support atHome clientsin the community. Support at Homeenablesin-homeagedcare clientsto remain
protected,independentand
engaged within their own homes.
n
Your responsibilities will include:
n
n
Conducting functional assessments and documenting client needs in detailed reports.
n
Communicating identified needs with clients, families, and service providers, and
coordinatingappropriate
referralsto relevant health professionals or specialists.
n
Prescribing suitable equipment and recommending home modifications to support client independence and safety.
n
