About the Role
n
We're seeking an experienced and motivated Roaming Care Manager to lead the delivery of high-quality, person-centred care across our services.
n
This is a full-time role with flexibility to be based remotely; however, regular and frequent travel is a key requirement, with our facilities located across TAS, VIC, NSW, WA, and QLD. You will be required to work across multiple sites, providing on-the-ground leadership and support where it is needed most.
n
In this role, you will work closely with residents, families and care teams to ensure care is secure,
responsive and tailored to individual needs. As a Roaming Care Manager, you will provide hands-on leadership, guiding best practice, supporting teams and ensuring clinical standards are consistently met across all locations.
n

As Roaming Care Manager, you will:
n
n
Lead and contribute to a collaborative team environment to ensure high-quality, person-centred care is delivered to residents and their representatives.
n
Mentor, support, and guide staff, promoting excellence in clinical care, lifestyle services, and team capability development.
n
Ensure duty of care is upheld by adhering to organisational policies, professional codes of conduct, ethical standards, and legislative requirements.
n
Monitor, evaluate, and report on clinical practices,
ensuring alignment with best practice standards, governance frameworks, and compliance requirements.
n
Participate in quality systems including audits, reviews, data analysis, and continuous improvement initiatives to enhance care outcomes.
n
Oversee care planning processes in collaboration with residents, families, and clinical teams, ensuring responsive and individualised care delivery.
n
Contribute to staff development through training, performance reviews, mentoring, and support of education and capability-building initiatives.
n
Maintain effective working relationships with internal teams, external providers, and community stakeholders while remaining responsive to operational needs, including after-hours availability when required.

About You
n
You'll thrive with us if you have:

n
Bachelor Degree in Nursing (Registered Nurse) for aged care facilities, or Nursing Diploma (Enrolled Nurse) for Victorian-based hostels.
n
Current registration with the Australian Health Practitioner Regulation Agency (AHPRA). Minimum 3 years' experience in residential aged care.
n
Strong working knowledge of Aged Care Quality Standards, Aged Care Act principles, documentation standards, residents' rights, and relevant funding instruments and legislation.
n
Well-developed clinical knowledge and skills aligned with contemporary nursing practice.
n
Strong communication and interpersonal skills with the ability to engage effectively with residents, families, and staff.
n
Current National Police Certificate (or
willingness/ability
to obtain).
n
Previous experience in nursing leadership and/or involvement in accreditation processes (desirable).
