Our services have a strong focus on prevention and early intervention and enable access to safe accommodation options, Holistic case management, community outreach programs and programs that teach vital independent living skills.
Our centres are safe and supportive environments creating an ideal environment for growth, where participants feel believed in, valued, listened to, and respected.About the roleAs the Client Services Coordinator, you will lead a dedicated team of case managers delivering high-quality support to young people aged ***** who are experiencing or at risk of homelessness, ensuring services are aligned with the Journey to Independence Model and Psychologically Informed Environment (PIE) framework.
Working across our refuge in St Kilda and supporting case managers in their outreach support across the Inner and Middle South regions of Melbourne, you will drive service excellence through strong leadership, operational oversight, stakeholder collaboration, and a commitment to achieving positive outcomes for young people.Reporting to the Program Manager, this is a permanent full-time position, based in office in St Kilda,
VIC.ConditionsCompensation is in accordance with the SCHADS Award Level 6.How you will make an impactLead and provide supervision to a team of 6 case managers to deliver high-quality, client-centred services for young people experiencing or at risk of homelessness both in our refuge and through outreach support in the communityBe responsible for providing daily leadership, management and coordination of services delivered across the Upton Rd refuge site and ensure continuity of support across 24/7 service delivery in conjunction with the after-hours coordinator and after-hours support worker rolesMonitor client outcomes, service performance, and case management practice, providing guidance and support on case planning, supporting PIE-informed engagement with supporting young people towards their goalsSupport and role-model proactive risk management and support ongoing quality and improvement actions across the service in conjunction with the leadership teamFoster strong partnerships with internal and external stakeholders to enhance service coordination, support pathways, and outcomes for young peopleDrive continuous improvement through staff development, reflective practice, quality assurance, compliance monitoring, and the analysis of service data and emerging sector trendsWhat you will bringTertiary qualification in Social Work,
