About the Opportunity
We are seeking a passionate and proactive Leisure & Lifestyle Assistant to join our Residential Services team in Templestowe. This role plays a vital part in enhancing the wellbeing of our residents by delivering engaging, meaningful and culturally responsive lifestyle programs that promote connection, independence and dignity.
As the Leisure & Lifestyle Assistant, you will lead and oversee the development, implementation and evaluation of reablement, rehabilitation and psychosocial programs, ensuring activities are person-centred and reflect the individual interests, preferences and capabilities of our residents.
The day-to-day responsibilities
- Support the consistent delivery of safe, person-centred, trauma-aware and healing-informed health and wellbeing programs aligned with human rights principles.
- Plan, organise and deliver a diverse range of meaningful leisure, lifestyle and wellbeing activities, including group programs, individual activities, excursions, special functions and celebrations.
- Encourage and support residents to participate in activities that reflect their individual interests, preferences, capabilities and cultural needs, with particular focus on residents living with dementia or at risk of social isolation.
- Promote independence,
choice, safety and social connection by supporting residents to maintain relationships with family, friends, community groups and external social opportunities.
- Support residents and their representatives to participate in wellbeing planning and shared decision-making, promoting autonomy and informed choice.
- Assist in completing, maintaining and reviewing Leisure & Lifestyle Assessments, My Life Story, Care Plans and other relevant documentation in collaboration with Clinical and Allied Health teams.
- Observe residents during activities, identify changes in their needs, behaviours or wellbeing, and accurately document observations in progress notes and care documentation.
- Work collaboratively with clinical, care and allied health teams to ensure leisure and lifestyle activities contribute positively to residents' overall wellbeing and care outcomes.
- Ensure wellbeing activity plans are followed and provide feedback to the Coordinator regarding resident participation, program effectiveness and opportunities for improvement.
- Respond to routine feedback from residents and families in a professional manner and escalate concerns to the Coordinator where required.
- Support the coordination and delivery of external wellbeing programs, including Creative Arts Therapy Australia (CATA), by assisting with preparation, resident participation and follow-up.
What success looks like for this role:
- Certificate IV in Leisure & Health (Lifestyle & Leisure) or equivalent (essential).
- Current First Aid Certificate.
- Demonstrated experience in aged care within a Leisure & Lifestyle or similar role.
- Strong understanding of person-centred care, wellness and reablement approaches.
- Excellent communication and interpersonal skills, with the ability to build meaningful relationships.
- Ability to lead, motivate and support team members.
- Strong organisational skills with the ability to plan, coordinate and evaluate programs.
- Experience working with culturally diverse communities.
- Greek language skill preferred.
