About the role

Join the Aged Care Assessment (ACA) team at The Royal Melbourne Hospital (RMH), Royal Park campus, and help shape quality aged care services for older Australians across Melbourne's Northern and Western Metropolitan regions.

As a Team Leader, you will provide operational and clinical leadership to an interdisciplinary team of clinical and non-clinical assessors, supporting the delivery of high-quality triage, assessment, care planning, delegation, training and quality activities.

You will lead a portfolio across areas such as intake, assessment delivery, support plan reviews, training or quality, while providing clinical guidance for complex assessments and supporting team performance. You will also maintain a hands-on clinical component, contributing to triage, delegation and assessment activities.

This is an opportunity to combine people leadership, clinical expertise and service improvement while supporting older Australians to access the care and services they need.

Key Accountabilities

- Provide operational and clinical leadership to the interdisciplinary workforce of clinical and non-clinical assessors to deliver the range of assessment functions including Triage, Assessment, Care planning, Delegation, Training and Quality
- Portfolios include intake, assessment delivery,



support plan review, and training and quality.
- Support team members to manage complex assessments
- Participate in direct care delivery including triage, delegation, reviews and assessment tasks

About You

You are an experienced health professional and people leader who offers:

- Relevant Allied Health, Nursing or related qualification aligned with Commonwealth guidelines.
- Training qualification, such as Certificate IV in Training and Assessment or higher.
- Minimum five years' clinical experience, including aged care assessment, intake, triage, delegation and reviews.
- Experience leading community teams, managing performance and building workforce capability.
- Robust clinical, communication and digital skills, with a person-centred approach.
- Ability to work independently and collaboratively within a multidisciplinary team.
