- Position Title: 2x Aged Care Engagement Officer
- Duration: Part time, fixed term until 30 June 2027 (30.5 hours/week)
- Base Salary: $72,413 to $77,931 per annum (HPCS 2.1 to 2.4)

About the role
Danila Dilba Health Services is seeking for a motivated and compassionate Aged Care Engagement Officer who plays a vital role in building trusted relationships with local Elders, older Aboriginal and Torres Strait Islander people, and their families. This position is focused on increasing awareness and understanding of aged care and ensuring individuals are supported in accessing culturally appropriate aged care services. Working under the supervision of the Coordinator Elder Care Support Program, the Aged Care Engagement Officer is responsible for coordinating and sequencing client support to facilitate smooth and effective access to both internal and external aged care services.
Responsibilities:
- Provide active community outreach to promote aged care services, identify and engage with potential clients.
- Provide pre-assessment support and assist consumers in navigating aged care system and access pathways.
- Assist clients with advocacy and support throughout the My Aged Care registration and review processes.
- Support clients to identify goals and needs to ensure they get the appropriate level of care and services, which can include referrals to other specialised services and support.
- Support clients through the aged care assessment process and support plan development.
- Guide consumers and families in selecting aged care providers and understanding of contractual and funding arrangements.
- Maintain up-to-date knowledge of local services and engage with My Aged Care and assessment staff collaboratively.
- Liaise with the aged care team to continuously review and enhance the role.
To be successful you will need:
Essential
- Aboriginal or Torres Strait Islander descent.
- Excellent and robust relationships with the local Aboriginal and Torres Strait Islander Community.
- Good communication and interpersonal skills, with the ability to build trusting relationships in community.
- Experience working with older people.
- Understanding of the internal ACCHO services available to clients.
- Knowledge of or willingness to learn about the aged care system and landscape.
- Ability to or willingness to learn how to enter data and complete reporting.
- Demonstrated experience working unsupervised and as part of a team.
Desirable
- Qualifications in Aged care or Community Services.
- Experience in working in an Aboriginal Community Controlled Health Service.
- Experience working in community setting.
- Demonstrated understanding of the principles of person-centred care and trauma-informed practice.
