About the role The Care Partner plays a central role in delivering Jewish Care Victoria’s commitment to person-centred, goal-directed care that enables older people to live independently and meaningfully in the community. This role is the key point of continuity for each client, providing care management, service coordination, referrals, and ongoing monitoring across all aged care funding streams, including Support at Home, CHSP, Claims Conference, and Private Services.
As part of the Care Partnering and broader Community Aged Care Team, the Care Partner works collaboratively with other Care and Clinical Care Partners, Clinical Leads, internal teams, and external providers to ensure that each individual’s care journey is seamless, culturally responsive, and aligned to their goals and changing needs. Care management is delivered in a flexible and responsive manner, with a care management activity completed at least monthly and the care plan reviewed at least every 12 months, or whenever the client’s needs, services or circumstances change. The role involves proactively identifying unmet needs and initiating appropriate referrals to internal or external supports.
While formal qualifications are not mandatory, the role requires a willingness to engage in upskilling, experience in aged care and a strong commitment to teamwork and respect for client dignity, autonomy, and wellbeing.
What We're Looking For
- Strong care coordination, communication and time management skills
- Ability to quickly build rapport with older people and their families
- Understanding of aged care funding programs and care planning processes
- Experience working with older people in a community-based or in-home setting
- Experience supporting vulnerable clients and diverse communities
- Sound judgement when responding to client needs, risks and competing priorities
- Empathy, cultural respect and a commitment to person-centred care
- Russian, Polish, Hebrew or Yiddish language skills (desirable, but not essential)
- Certificate IV in Aged Care or Community Services or equivalent experience (highly regarded)
