The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).
We are seeking a dedicated and compassionate Child and Family Health Nurse to provide family-centred care and support to families with children aged 0-5 years. In this rewarding community-based role, you will deliver Universal Health Home Visits, developmental and health surveillance, screening, and the early identification of health, developmental and family vulnerabilities.
Working collaboratively within a multidisciplinary team, you will promote positive health and wellbeing outcomes through evidence-based practice, timely intervention, advocacy and culturally responsive care. You will also contribute to a supportive learning environment by providing clinical guidance, mentoring and preceptorship to staff and students.

What you will bring to the role
- Must be of Aboriginal and/or Torres Strait Islander descent (applicant's race is a genuine occupational qualification and is authorised by Section 14 of the Anti-Discrimination Act 1977, NSW).
- Current registration as Registered Nurse Division 1 with AHPRA and demonstrated recent experience/clinical skill in provision of care to children aged 0-5 years and their families/carers as a registered nurse (including completion of a new graduate RN program or 12 months full-time equivalent experience working as a Registered Nurse).
- Demonstrated ability to effectively and sensitively liaise with Aboriginal and Torres Strait Islander clients and the local Aboriginal communities.
- Demonstrated evidence of working within a multidisciplinary team to facilitate the care planning for families with complex needs.
- Demonstrated ability to organise and prioritise work and an ability to effectively work both within a team and autonomously. Demonstrated effective communication (written and verbal) and interpersonal skills.
- Demonstrated commitment to professional development, evidence-based practice and an awareness of changes within the health care environment.
- Demonstrated knowledge of and ability to utilise information technology systems within a variety of applications (e.g. Cerner) and commitment to undertake training in electronic health record systems as required.
- Current NSW State driver's licence (including P1 or above) and willingness to use for work purposes. This may include driving long distances across the LHD.
