The Kids Hub is a free, inclusive mental health and wellbeing service supporting children aged 0–12 across the City of Swan who are experiencing mild to moderate mental health challenges.
The first service of its kind in WA, The Kids Hub brings together specialist organisations to provide accessible, early intervention support in a safe, nurturing and child-focused environment. The Kids Hub is a free service with a no-wrong-door approach, providing step-up, step-down support for children and their families for as long as they require.
Through personalised assessment, therapy and wellbeing support, The Kids Hub takes a holistic approach to children’s mental health, ensuring children and families can access the right support early, regardless of their circumstances.
About the Role The Child and Family Care Navigator plays a pivotal role in the exciting, multidisciplinary team at the Midland Medicare Mental Health Kids Hub. As the first point of contact for children and families, they are the heart of the Hub, providing essential guidance and support as families navigate mental health and social systems. Working closely with children and families, the Child and Family Care Navigator helps identify how the Kids Hub can best support them, ensuring their journey is smooth and tailored to their needs.
Accompanying families across all three pillars – Community Development, Family Support, and Specialist Therapeutic Support – the Child and Family Care Navigator uses a step-up,
step-down model to provide flexible, comprehensive support at every stage. You will also be required to facilitate groups and psychoeducation for children and families, alongside the assessment of needs.
This service provides a safe and welcoming space for all children across the City of Swan. With operating hours from 8 AM to 7 PM Monday to Friday. The role will work between the hours of 8:30-7pm (up to 7.6 hours per day).
The role entails a combination of community-based outreach, in-home and office-based models of working. This is a unique chance to make a real difference in the lives of local children.
