About the Role
We're seeking an experienced and passionate Care Coordinator to join our leadership team at Vermont Views Care Community.
This is a key clinical leadership role responsible for coordinating resident care, providing hands-on clinical oversight, supporting care teams, and ensuring residents receive safe, high-quality, person-centred care. Working closely with the General Manager and Care Manager, you'll play an important role in driving clinical excellence, supporting team capability, and enhancing resident outcomes.

Sector-leading systems & innovation Who We're Looking For:
We welcome applications from experienced aged care clinicians with: Current AHPRA registration as a Registered Nurse (Division 1)
Demonstrated experience in care coordination, clinical leadership and aged care
Strong understanding of Aged Care Quality Standards and clinical governance
Proven ability to lead, coach and support multidisciplinary teams
Experience coordinating assessments, care planning and clinical reviews
Excellent communication and stakeholder engagement skills
Strong documentation, compliance and care management capability
Basic Life Support Certificate
