As a Community Health Nurse, you'll provide vital primary health care, outreach support and health education, working alongside a multidisciplinary network of services to improve access to care and create positive health outcomes for young people across the Barwon and Colac regions.
As a Community Health Nurse, you will:
- Provide assertive outreach, health assessments and primary health care support to young people experiencing or at risk of homelessness.
- Deliver health promotion, brief interventions, sexual and reproductive health support, and referrals tailored to individual needs.
- Collaborate with GPs, homelessness services, allied health professionals and community partners to coordinate integrated care and improve health outcomes.
- Maintain accurate clinical records, contribute to program development and evaluation, and uphold best-practice nursing and quality standards.
Position Details
- Full-time, fixed term position until 30 September 2027
- 1 FTE working 38 hours per week
- $108,284 p/a + Super
What We’re Looking For
We value individuals who bring dedication, empathy, and expertise to their roles. To succeed in this position, you will have:
- Tertiary qualifications and current AHPRA registration in Nursing (Registered Nurse) preferably with completed Nurse Vaccination Training, Sexual and Reproductive Health Training, Mental Health First Aid and/or ASSIST Training
- Demonstrated experience in working with young people in a homelessness and/or primary health care setting.
- Comprehensive and practical understanding and ability to manage complex health presentations with psychosocial issues and including moderate to acute mental health and / or Alcohol and Other Drug issues.
- Ability to understand and support clients via assessment to manage supports, connections and risk associated with homelessness and/or family violence.
