Community Nurse – Home Dialysis Service

Part time fixed term until January 2027 | 32 hours per fortnight

Work within a supportive, specialist renal team committed to patient independence

Monday to Friday, with no weekend or public holiday rostered duties.

About the role The Royal Melbourne Hospital is one of Victoria's leading public health services, renowned for excellence, innovation and compassionate care. We are currently seeking an experienced Registered Nurse to join our Kidney Care Home Dialysis Service (HDS) at the Royal Park Campus.

This is a rewarding opportunity to support patients and families who have chosen the independence and benefits of home renal replacement therapy, including peritoneal dialysis and haemodialysis

Key Accountabilities

Provide patient and family-centred renal education, training and clinical support.

Empower patients to safely and confidently manage dialysis at home.

Provide case management and ongoing clinical support across the patient journey.

Deliver outreach clinics and home visits across metropolitan and regional/rural settings.

Work collaboratively with nephrology teams, GPs, community groups and external stakeholders.





Support continuity of care through effective coordination and stakeholder engagement.

Participate in overnight regional and rural visits as required.

About You

You are an experienced renal nurse with strong clinical assessment skills and the ability to work autonomously across hospital and community settings. You will bring:

Postgraduate qualification in Renal Nursing or relevant critical care specialty

Minimum 2 years’ current renal nursing experience

Strong clinical assessment, time management and organisational skills

Excellent communication, negotiation and interpersonal skills

Ability to provide patient education and support self-management

Current Australian driver’s licence, Basic Life Support and Microsoft Office skills

Experience in acute renal or community nursing and supporting change is desirable
