Sunridge Support Solutions is seeking a reliable, patient and compassionate Support Worker to provide community access support to a participant who is a wheelchair user, blind and living with an acquired brain injury (ABI).The successful Support Worker will assist the participant to safely access the community, participate in activities they enjoy, build confidence and maintain independence and social connection.

- Support the participant to access the community safely and confidently.
- Assist with outings, appointments, shopping, social activities and recreational activities.
- Provide protected wheelchair mobility and assistance where required.
- Provide appropriate guidance and communication for a participant who is blind.
- Support the participant with navigating unfamiliar environments and managing potential hazards.
- Use clear,



calm and respectful communication.
- Follow the participant's support plan, preferences and routines.
- Monitor for changes in mood, behaviour, fatigue or wellbeing and report concerns appropriately.
- Encourage independence and participation rather than doing everything for the participant.
- Maintain professional boundaries and respect the participant's dignity, privacy and choices.
- Complete accurate progress notes following each shift.
- Follow NDIS, WHS, incident reporting and organisational policies and procedures.

Pay: $45.28 – $63.39 per hour Expected hours: No less than 20.0 per week
