About the Role

Hemingway Support Pty Ltd is a registered NDIS provider committed to delivering high-quality, person-centred support services to individuals living with disability. We are currently seeking compassionate and dedicated Disability Support Workers to join our growing team on a full-time basis.

Key Responsibilities

- Provide personal care and daily living assistance to participants, including support with hygiene, dressing, meal preparation, medication administration, mobility, and household tasks.
- Deliver person-centred support services in accordance with individual care plans, promoting participants' independence, wellbeing, dignity, and achievement of personal goals.
- Support community access and social participation by accompanying participants to appointments, recreational activities, shopping, and other community engagements.
- Provide in-home and Supported Independent Living (SIL) support while maintaining a safe, inclusive, and supportive environment for participants.
- Monitor and report participants' health, behaviour, and wellbeing, and maintain accurate records, progress notes, and incident reports in accordance with organisational policies and NDIS requirements.
- Collaborate effectively with families, carers, allied health professionals, support coordinators, and other stakeholders to ensure high-quality and coordinated service delivery.





Skills and Experience
- Certificate III in Individual Support (Disability), Aged Care or a related qualification, or 1 year full time relevant work experience,
- Strong understanding of person-centred practices and the NDIS framework, with the ability to implement individual support plans and promote participant independence and wellbeing.
- Effective written and verbal communication skills, including the ability to maintain accurate records, prepare progress notes, and communicate professionally with participants, families, carers, and healthcare professionals.
- Ability to work independently and collaboratively within a multidisciplinary team, while building positive relationships with participants and stakeholders.
- Robust commitment to workplace health and safety, the NDIS Code of Conduct, and relevant legislative and organisational requirements, ensuring safe and high-quality service delivery.
- Current NDIS Worker Screening Check, Working with Children Check, First Aid and CPR Certificate, and a valid Australian driver's licence with access to a reliable vehicle.
