Job Description

Assistly is seeking an experienced Level 2 Support Coordinator to join our Braybrook, VIC team on a contract basis. You will independently manage a Level 2 Support Coordination caseload, helping NDIS participants understand and implement their plans and connect with appropriate services.

Applicants must have practical NDIS Support Coordination experience and be confident coordinating multiple providers, families and plan managers to achieve participant goals and outcomes.
