We are a locally owned and operated provider specialising in Specialist Support Coordination, Support Coordination, Psychosocial Recovery Coaching, and Social Work - and that’s all we do. About the role As a Support Coordinator you will:
- Navigate and coordinate services across multiple systems
- Advocate for participant rights, safety, and quality of life
- Build capacity and sustainable support networks
- Work collaboratively with families, providers, and stakeholders
- Work from home and in the community - full drivers license and comprehensively insured roadworthy vehicle essential
What we’re looking for
- Experience in Support Coordination or transferable skills
- Strong understanding of the NDIS and broader service systems
- Ability to work within complex, high-risk, or safeguarding contexts
- Excellent communication, documentation, and organisational skills
- A genuine commitment to ethical, rights-based practice
- Relevant qualifications (preferred)
