Program Facilitator & Outreach Officer

At Pathways to Independence, we’re looking for an energetic, people-focused Program Facilitator & Outreach Officer who can inspire participants, build strong community partnerships, and help expand opportunities for people living with disability.

About the Role

As our Program Facilitator & Outreach Officer, you’ll play a key role in delivering engaging capacity-building programs while helping connect participants with opportunities that support independence, confidence, and community participation.

The role combines frontline program delivery with proactive outreach activities designed to strengthen referral pathways, increase participant enrolments and promote Pathways services throughout the community.

What is the Pathways to Independence Program?

Pathways to Independence is a structured, centre-based group program designed to support participants to build confidence, independence and practical life skills aligned with their NDIS goals. The program focuses on areas such as:

- Communication and social skills
- Emotional regulation and self-awareness
- Daily living skills and routine-building
- Confidence, independence and community readiness

Every day, you’ll inspire participants to develop practical skills, connect with their community, achieve their goals and discover what they’re truly capable of

Key Responsibilities

Program Facilitation & Participant Support

- Deliver engaging workshops and participant activities
- Support participants to achieve goals, build confidence and increase independence
- Create positive, inclusive and secure learning environments
- Encourage community participation and real-world learning experiences




- Provide transport support and travel training where required
- Celebrate participant achievements and support personal growth

Outreach, Recruitment & Marketing

- Build strong referral partnerships with Allied Health professionals, schools and community organizations
- Follow up participant enquiries and referrals
- Promote Pathways programs within the community
- Attend networking events, expos and community activities
- Contribute to participant growth and enrolment initiatives
- Be a positive ambassador for Pathways

About You

- Experience in disability, community services, youth work, education or a related field
- Experience facilitating groups, workshops or participant activities
- Experience with marketing and outreach
- Robust relationship-building and communication skills
- Confidence networking and engaging with community stakeholders
- A proactive approach and the ability to work independently
- Excellent organization and documentation skills
- A positive, solutions-focused attitude
- A passion for helping people reach their goals and participate fully in their community

Qualifications & Requirements

- Certificate IV or higher in Community Services, Disability, Youth Work, Human Services, Education, or related discipline (or equivalent experience)
- Current Victorian Driver License
- NDIS Worker Screening Check
- Working With Children Check
- Current First Aid & CPR Certification
- Ability and willingness to provide participant transport support
