About The Role Sydney Local Health District has a long and proud history of delivering care in the community including prevention, early intervention, assessment, treatment, health maintenance and continuing care services designed to improve and support the health and wellbeing of individuals and communities.

The Community Mental Health/Core Team model of care states:

The “Community Mental Health Team” (Core Team) is the mainstay of the community mental health system. Core Teams and provide a range of clinical services including intake, assessment, consultation, treatment ,and care coordination. Care coordination is the main component of this model of care and includes extended care coordination, targeted packages of care, and short term care coordination.

Consumers under the care of Core Teams will either have:

- Short term interventions and may be discharged or referred back to their GPs or other treatment providers after a period of weeks or months.
- A period of treatment as recommended by evidence based practices.
- Ongoing care and monitoring for prolonged periods.





The Mental Health Nurse will practice in keeping with the standardised Sydney LHD position description for the profession in addition to this position description. For more information, please view the Position Description

Ideal Candidate

- Current nursing registration with AHPRA.
- Competencies to provide contemporary mental health practice.
- Understanding of and commitment to recovery oriented services and consumer involvement in service provision.
- Ability to develop and maintain effective relationships with GPs, Service partners, and external providers.
- Ability to participate in a non-rotating roster.
- Current NSW Drivers Licence (P1 and P2 acceptable).
