About The Role Carinity Home Care – Brisbane South and Logan are looking for a Registered Nurse to join their team. You will provide clinical and personal care support to Carinity Home Care Clients. Your approach to care is holistic taking in account the physical, social and spiritual needs of the client, adopting a problem-solving approach for the planning, delivery and evaluation of care provided. Your Day-to-day Will Involve
- Delivering person-centred care providing high-quality clinical care to the community
- Ensuring care plans are regularly reviewed and updated with collaboration with the multidisciplinary team
- Managing medications and supplies in line with legal requirements
- Ensuring compliance with aged care standards and policies
- Promoting a safe, respectful, and inclusive setting within the community and staff
- Building effective relationships with clients, families, and external stakeholders
About You You bring warmth, patience and genuine empathy to your work, along with a strong commitment to delivering high quality care to older people. Confident in coordinating services and working collaboratively, you build trusting relationships with clients and colleagues. Your strong communication skills and client-first approach support wellbeing, independence and positive experiences.
- Minimum two years recent experience as RN
- Demonstrated clinical skills and experience working within the Community and Aged Care.
- Experience in wound care and catheter management in a community-based healthcare environment is desirable
- Ability to work in a multidisciplinary team and communicate effectively.
- Current registration as a Registered Nurse with AHPRA.
- Current Driver’s Licence and own transport
- Excellent time and IT management skills
