About the Role
We're looking for a Registered Social Worker to join AusUnity Aid on a subcontract basis, 3 days per week, providing case management support to clients across community, hospital, and correctional settings.
This is a meaningful opportunity for a social worker who is confident working across multiple systems and settings, and who is skilled at building trust and rapport with clients, and with the allied health and multidisciplinary teams supporting them.
The role includes regular travel to prisons and hospitals to conduct client visits, assessments, and care coordination, so we're looking for someone comfortable and experienced working within institutional and clinical environments.
Engagement type: This position is offered as an independent contractor arrangement. The successful applicant will invoice AusUnity Aid under their own ABN and is responsible for their own tax, superannuation, and insurances.
What You'll Do
- Conduct client intake, psychosocial assessments, and case management, including for clients in custodial (prison) settings and hospital settings
- Visit clients in prisons and hospitals to provide continuity of care, discharge/release planning, and transition support back into the community
- Build and maintain strong working relationships with allied health professionals (e.g. psychologists, occupational therapists, nurses, GPs, psychiatrists) and other multidisciplinary team members involved in client care
- Develop, implement, and review individualised support and case management plans
- Liaise with correctional staff, hospital staff, families, and carers to coordinate holistic, wraparound support
- Identify and respond to risk, including safeguarding, duty of care, and mandatory reporting obligations
- Maintain accurate, timely, and confidential documentation and case notes
- Refer clients to appropriate internal and external services and supports
- Participate in supervision, case reviews, and interagency meetings
About You
- Bachelor or Master's degree in Social Work (AASW-accredited)
- Current membership with the Australian Association of Social Workers (AASW), or eligibility for registration
- Experience working within hospital, correctional, forensic, or justice-related settings highly regarded
- Demonstrated ability to build rapport quickly across diverse settings and with multidisciplinary and allied health teams
- Sound understanding of relevant legislation, including privacy, safeguarding, and mandatory reporting
- Strong assessment, case management, and report-writing skills
- Resilience and confidence working within secure or high-need environments
- Current National Police Check, and ability to obtain prison/corrections site clearance
- Current NDIS Worker Screening Check (or willingness to obtain)
- Current Working with Children Check (if applicable)
- Valid ABN, and appropriate Professional Indemnity and Public Liability Insurance
- Current driver's licence and access to a reliable vehicle for site visits
