Care Implementation: Execute and monitor patient care plans while documenting and communicating status changes to the team. Care Coordination:



Act as the primary liaison across clinical disciplines to ensure integrated and seamless patient care. Accurately transcribe and carry out all physician and nursing orders to facilitate treatment. Discharge Planning: Assess and coordinate post-care requirements with the healthcare team to ensure secure patient transitions. Licensure / Certification / Registration: Registered Nurse credentialed from the Tennessee Board of Nursing obtained prior to hire date or job transfer date. Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
