Job Description

APM is seeking a Clinical Aged Care Assessor to evaluate needs of older Australians and connect them with services to support independent living. Daily travel within the service area is required to meet client needs.

You will conduct thorough assessments, refer to essential services, promote functional independence, and collaborate with families and carers while ensuring compliance with health contract requirements. Training and career development are provided.
