About The Role Join the Royal Melbourne Hospital’s specialist Neuropsychiatry team , recognised nationally and internationally for excellence in clinical care, research and innovation. This is an opportunity to make a meaningful difference to patients and their carers while working in a specialist service committed to improving outcomes and developing the future workforce. As a Grade 4 Social Worker, you’ll provide advanced clinical expertise to patients with complex neuropsychiatric presentations, working closely with the inpatient team and broader multidisciplinary service. You’ll contribute to psychosocial assessment, care planning, discharge planning, advocacy and connecting patients and carers with appropriate supports and services.
Key Accountabilities
- Provide advanced social work assessment, intervention and support to patients with complex neuropsychiatric presentations and their carers.
- Work closely with the inpatient and multidisciplinary teams to support person-centred care and discharge planning.
- Advocate for patients and connect them with appropriate health, community and social supports.
- Provide clinical leadership, supervision and guidance to colleagues and students.
- Contribute to education, research, quality improvement and service development.
- Build strong partnerships with patients, carers and internal and external service providers.
About You You are a qualified Social Worker with AASW membership or eligibility, bringing:
- Clinical experience supporting people with complex mental health, neurological or chronic health needs.
- Strong counselling, psychosocial assessment and advocacy skills, including NDIS experience.
- Ability to work autonomously and collaboratively within a multidisciplinary team.
- Strong clinical reasoning, problem-solving and discharge planning skills.
- Excellent communication and relationship-building skills with patients, families and carers.
- A commitment to person-centred, high-quality care and positive patient outcomes.
