The Sydney Children’s Hospitals Network is seeking a Youth Peer Support Worker to help young people transition from acute inpatient care to community services. You’ll collaborate with a multidisciplinary team, drawing on lived experience to engage with clients and clinical staff and to support recovery goals.
Using the CHIME framework, you will provide non-clinical support, empower youths and contribute to models of care while helping families access community resources and follow-up supports.
