What you'll do:

n

Deliver high-quality, person-centred nursing care in accordance with organisational policies, ANMC standards, and professional codes

n

Conduct holistic assessments in home or community settings and develop individualised care plans to promote independence and wellbeing

n

Coordinate, implement, and evaluate care plans to achieve optimal client health outcomes in the community

n

Work collaboratively with multidisciplinary teams, external providers, and community services to support client needs

n

Monitor client health status and escalated concerns or changes in condition appropriately

n

Maintain accurate,



timely, and compliant clinical documentation in line with organisational and regulatory requirements

n

Practise safely within scope, ensuring accountability and adherence to policies, procedures, and community care standards

n

Maintain up-to-date clinical knowledge and skills relevant to community-based care through ongoing professional development

n

Provide leadership, supervision, and support to Enrolled Nurses, support workers, and care staff

n

Assist with staff orientation, education, and ongoing competency development

n

Support student placements and assist with onboarding of agency or new staff

n

Contribute to continuous improvement, service development, and quality care initiatives

n

Communicate effectively with clients, families, carers, and stakeholders to support coordinated care

n

Promote client independence, health education, and self-management strategies

n

Escalate incidents, risks, and safeguarding concerns in line with reporting requirements

n

Act as a senior clinician in the community setting when required, including responding to urgent situations

n

Ensure compliance with quality standards, accreditation requirements, and Work Health & Safety practices in community environments

n

Identify and manage risks within clients' homes and community settings to maintain safe care delivery

n

Undertake additional duties within scope to support effective and responsive community care services.

n

Current AHPRA registration with no conditions

n

National Police Clearance

n

Current NDIS Worker Screening Check

n

First Aid Certificate / CPR

n

Ability to work efficiently under pressure.
