APD Mental Health Support Services is seeking an experienced and compassionate NDIS Support Coordinator to join our team on a maternity leave contract.

This position is expected to commence 12/10/26 and will initially be offered for approximately 6-12 months, with the possibility of ongoing work following the contract period.

APD Mental Health Support Services is a locally owned organisation supporting people with psychosocial and other disabilities throughout the Albury–Wodonga region. We pride ourselves on providing genuine, person-centred support and building strong, respectful relationships with our participants and their support networks.

About the role

The successful applicant will manage a varied caseload and assist participants to understand and implement their NDIS plans, connect with suitable services and work towards their individual goals.

Key responsibilities will include

- Developing trusting and professional relationships with participants
- Supporting participants to understand and make effective use of their NDIS plans
- Connecting participants with appropriate mainstream, community and funded supports
- Coordinating services and communicating with families, providers and other stakeholders
- Monitoring participant budgets and service agreements
- Preparing progress reports, change-of-situation requests and supporting documentation
- Assisting participants with plan reassessments and navigating NDIS processes
- Identifying and responding appropriately to risks, gaps in support and crisis situations
- Maintaining accurate,



timely and confidential case notes and records
- Working collaboratively with the APD team while managing your caseload independently

About you

We are looking for someone who is organised, approachable and genuinely committed to advocating for people with disability. The ideal applicant will have:
- Previous experience in NDIS Support Coordination
- A sound understanding of the NDIS, participant plans and relevant reporting requirements
- Experience supporting people with psychosocial disability and complex needs
- Solid written and verbal communication skills
- Excellent organisation, time-management and problem-solving abilities
- Confidence communicating with participants, families, providers and government agencies
- The ability to work both independently and as part of a supportive team
- A person-centred, trauma-informed and recovery-focused approach

Essential requirements

- Relevant qualification in community services, mental health, disability, social work or a related field
- Current NDIS Worker Screening Check
- Current Working with Children Check
- Current First Aid and CPR certification
- Current Australian driver’s licence
- Reliable, insured vehicle
- Australian working rights
