Disability Support Worker | Swan Hill VIC
We are looking for an experienced Disability Support Worker to support a middle-aged male participant based in Swan Hill.
This role is best suited to someone who has robust experience in Disability Supports and can provide a calm, supportive, and skilled approach while promoting independence.
About the Participant:
The participant is a middle-aged man with a history of mental health. He is relatively independent but requires consistent support to maintain his wellbeing, routine, and community engagement.
Key Responsibilities:
Community access support (outings, shopping, appointments)
Meal preparation and support with maintaining a healthy routine
Light domestic duties (cleaning, general household upkeep)
Transport to activities and appointments
Providing companionship and emotional support
Encouraging independence while offering appropriate guidance
Essential Requirements:
Female candidate (genuine occupational requirement)
Extensive experience supporting individuals with mental health
Relevant qualifications in Mental Health, Disability, Community Services, or similar
Current NDIS Worker Screening Check
Valid Working With Children Check (WWCC)
Current First Aid & CPR certification
Valid Driver’s Licence and reliable, insured vehicle
Robust communication and interpersonal skills
Ability to work independently and follow care/support plans
Desirable:
Experience working with NDIS participants
Understanding of recovery-oriented and person-centred approaches
