Position Profile:
WA Country Health Service South-West is looking for experienced clinical pharmacists to join the Bunbury team.
These roles promote secure, rational and cost-effective medication therapy by providing clinical pharmacy servicesfor hospital patients throughout the South West region.This includes, but is not limited to, medication reconciliation for admitted hospital patients, monitoring the patient's total medication profile, dispensing medications to patients, providing drug information to nursing and medical staff and making independent decisions on therapeutic strategies to optimize patient care.Your practice as a Pharmacistwill alignto the Pharmacy Board of Australia Registration Standards, the Pharmacy Board of Australia Codes and Guidelines including the Pharmacy Code of Conduct for Registered Health Practitioners and WACHS policies and guidelines.This is a Permanent Full Time, Permanent Part Time, Fixed Term Full Time or Fixed Term Part Time appointment, with the possibility of extension(s) and/or permanency.
