About the Role We are seeking a compassionate, reliable and dedicated Aged or Disabled Carer to join our team and provide high-quality care and support to older people and people living with disability.
The successful applicant will provide person-centred assistance based on each client's individual needs, preferences, goals and care plan.
The role may involve providing support in clients' homes and within the community to promote independence, wellbeing, social participation and quality of life.
Key Responsibilities The Aged or Disabled Carer will be responsible for: Providing personal care and assistance with daily living activities in accordance with individual care plans.
Assisting clients with dressing, grooming, personal hygiene, mobility and other personal-care needs.
Supporting clients to maintain their independence and participate in everyday activities.
Providing assistance with meal preparation and other household tasks as required.
Assisting clients with shopping, appointments, transport and community activities.
Providing social support and companionship to clients.
Supporting clients to participate in recreational, social and community activities.
Providing respite and support to clients and their carers where required.
Assisting clients to develop and maintain daily living skills.
Supporting clients to achieve their individual goals and maintain their preferred lifestyle.
Monitoring and reporting changes in clients' physical, emotional or social wellbeing to the relevant supervisor or care coordinator.
Maintaining accurate and timely client records, case notes and relevant documentation.
Following individual care plans, organisational policies, procedures and relevant health and safety requirements.
Maintaining client confidentiality, privacy, dignity and respect at all times.
Working collaboratively with clients, families, carers, colleagues and other relevant service providers.
Supporting culturally safe, inclusive and respectful service delivery.
Undertaking other reasonable duties consistent with the position and level of responsibility.
Selection Criteria The successful applicant will demonstrate: A genuine commitment to supporting older people and people living with disability.
Previous experience in aged care, disability support, community care or a similar role.
Ability to provide person-centred care and support.
Good communication and interpersonal skills.
Ability to work independently and as part of a team.
Reliability, patience, empathy and a respectful approach to clients.
Ability to maintain professional boundaries and client confidentiality.
Ability to follow care plans, workplace procedures and relevant legislation.
Positive written and verbal communication skills.
Ability to complete required documentation and client records.
Flexibility to work across different shifts and service requirements, where applicable.
A current Australian driver's licence, where required for the position.
Qualifications and Mandatory Requirements The successful applicant will be required to hold, or be willing to obtain, the relevant qualifications and clearances required for the position.
Depending on the role and service area, requirements may include: Certificate III in Individual Support (Ageing and Disability) or equivalent relevant qualification.
Current NDIS Worker Screening Check , where working in disability services.
Current Working with Children Clearance (Ochre Card) , where applicable.
Current National Criminal History Check.
Current First Aid and CPR certification, where required.
Current driver's licence, where required.
Any other clearances, vaccinations or mandatory training required by Anglicare NT.
Anglicare NT states that employees working in disability services require an NDIS Worker Screening Check, while employment generally requires a current Ochre Card and recent National Criminal History Certificate.
