Community Support Worker Full – time | Salary range : $68,000 - $80,000 Summary: The position requires assessing individual and community needs, coordinating community support initiatives, facilitating access to health, housing, employment, education and welfare services, and working with service providers and community organisations to promote independence, inclusion and wellbeing.
Duties and responsibilities: · Assess each individual's needs and provide practical support to promote independence, wellbeing and access to appropriate community services and resources.
· Assist individuals to access healthcare, housing, social, employment, education and other community-based services.
· Support individuals in developing life skills, including budgeting, household management and social interaction.
· Encourage and assist individuals to engage in recreational, educational and social activities to promote community participation and social inclusion.
· Monitor and report on each individual's progress, maintain accurate records and identify changes in support needs or circumstances.
· Provide emotional support and advocacy, promoting individuals' rights, wellbeing and access to appropriate services.
· Liaise and collaborate with families, community organisations and other service providers to coordinate support and ensure individuals can access required services and resources.
· Maintain safety and confidentiality, ensuring that all interactions and activities comply with organizational policies and relevant regulations.
Qualifications and experience: · AQF diploma in the related field · At least one year of full-time (or part-time equivalent) work experience in community support, caregiving, or similar role.
Essential skills: · Strong communication and interpersonal skills, with the ability to engage with · participants and their families.
· Understanding of the needs of vulnerable individuals, including those with · physical or intellectual disabilities.
· Ability to work independently and as part of a team.
· Good problem-solving and organizational skills.
· Knowledge of community resources, health and safety guidelines, and advocacy principles.
Disability Support Worker Full – time | Salary range : $68,000 - $80,000 Summary:
The position requires providing person-centred care and support to individuals with physical, intellectual or developmental disabilities, assisting with daily living activities, promoting independence and helping participants achieve their personal goals in a safe and inclusive environment.
Duties and responsibilities: · Provide assistance with daily living activities, including personal care (e.g., dressing, grooming, bathing), mobility support, and meal preparation.
· Support participants with medication management and ensure that prescribed · medications are taken correctly and on time.
· Assist with recreational and community activities to encourage social participation and engagement.
· Promote independence by encouraging and supporting participants to complete tasks as much as possible.
· Monitor participants' well-being and report any concerns or changes in their condition to the supervisor or healthcare team.
· Document care activities, progress, and incidents in accordance with organizational policies.
· Foster positive relationships with participants, family members, and other support staff to ensure comprehensive care.
· Assist in implementing behaviour support plans and providing emotional support when needed.
Qualifications and experience: · AQF Certificate II and at least one year of full-time (or part-time equivalent) work experience · AQF Certificate III with relevant post qualification work experience of at least one year (minimum 20 hours plus per week) · Previous experience in community support, caregiving, or similar role is preferred.

· Ability to work effectively with people with a range of physical, intellectual, and developmental disabilities.

Day Service Worker Full – time | Salary range : $75,000 – $90,000 Summary: The position requires supporting individuals in structured day services by facilitating activities, promoting independence, community participation and skill development, and providing assistance with daily living, emotional and physical support.
Duties and responsibilities: · Facilitate and support daily activities, including recreational programs, life skills training, and community engagement.
· Provide person-centred support to participants based on their individual goals and care plans.
· Assist with personal care tasks such as toileting, dressing, and meal assistance where required.
· Encourage social interaction and community participation through group outings and programs.
· Monitor and document participant progress, behaviours, and achievements.
Collaborate with families, carers, and support professionals to ensure cohesive service delivery.
· Ensure safety and wellbeing of participants at all times, adhering to organisational policies and procedures.
· Assist in maintaining a clean, safe, and inclusive setting.
· Participate in regular training and professional development sessions.
Qualifications and experience: · AQF diploma in the relevant field · At least one year of full-time (or part-time equivalent) relevant work experience.
· Prior experience working with people with disabilities or in a community day program setting is highly desirable.
