Role Overview
Provide pastoral care coordination within HammondCare At Home, supporting clients, families, staff, and volunteers. Act as a role model for HammondCare's Christian mission and values, coach staff to meet spiritual needs, and deliver compassionate care in community settings.

Key Duties and Responsibilities
• Conduct spiritual and emotional needs assessments for clients and families upon service entry, in consultation with HCAH staff.
• Develop and document spiritual care plans in collaboration with staff to ensure client spiritual needs are acknowledged.
• Provide initial and ongoing spiritual and emotional support to HCAH clients and their families.
• Participate in case management reviews, contributing to the development and evaluation of care plans regarding spiritual and emotional needs.
• Contribute to carer support groups and other faith-supporting activities as appropriate.




• Liaise with relevant religious and spiritual leaders to coordinate spiritual support for clients and families as needed.
• Provide prayerful spiritual and emotional support to staff; distribute relevant materials, articles, and books.
• Write articles for Hammond Care publications as required.
• Offer Christian perspectives on ethical issues as required.
• Deliver staff learning in pastoral care areas (e.g., boundaries, grief and loss, faith and hope) as directed by the Pastoral Care Manager.
• Provide opportunities for staff to understand Hammond Care's Christian values and their application in services.
• Extend staff skills in meeting client spiritual needs, including identifying learning and development resources.
• Participate in staff meetings, in-services, expos, and other events to promote pastoral care opportunities and build partnerships.
• Regularly meet with local service managers and staff to receive feedback, identify needs, and report on changes or developments in the Pastoral Care service.

Qualifications and Required Experience
Essential
• Certificate IV in Pastoral Care or recognized equivalent qualification (e.g., Diploma of Chaplaincy, Theology degree). Candidates who do not hold the required qualification but have substantial hands-on experience and proven capability in a similar role will be considered.
• Minimum 3 years' experience in a pastoral care or chaplaincy role, preferably in aged care, community care, or a similar setting.
• Commitment to engage and align with Hammond Care's Christian Mission, Motivation, and Mission in Action.
• Solid personal active Christian faith and ongoing church involvement.
• Strong administrative and computer skills.
• Ability to work independently and as part of a team.
• Empathy and patience.

Desirable
• Ordination or credentialled ministry.
• Understanding of the spiritual and pastoral needs of those living with dementia and/or major health issues.
• Experience delivering training or coaching staff.
